Position available for a full-time Intake/Transition Specialist responsible for providing transition activities for youth in the residential program. This includes treatment, case management, and coordination of services for youth who are leaving Hephzibah. Also responsible for screenings and assessments for admission into the program.
The Intake Specialist is responsible for the initial screening and assessment of individuals seeking admission to our residential program. This role involves gathering and verifying client information, evaluating eligibility, and coordinating the admission process to ensure a smooth transition for new residents. The Intake Specialist will serve as the first point of contact for prospective residents and their families, providing them with information and support throughout the intake process.
The Transition Specialist will engage with children and families at time of admission and participate in discharge planning. The Transition Specialist will provide family therapy, parental coaching and help identify and problem-solve barriers to family engagement.
Requirements include:
- Master’s degree in social work, Psychology, or Counseling from an accredited college or university.
- Licensure preferred
- Experience in child development, crisis intervention, family therapy and work/life experience with children.
- Must be self-motivated and empathetic.
- Excellent written and verbal communication skills, strong organizational skills, ability to work independently and effectively with children, parents, staff, school personnel and oversight agencies.
- Sensitivity and responsiveness to the cultural differences in staff and client populations.
- Must have reliable transportation
